HOW TO APPLY FOR A LIBRARY CARD

When applying, please bring:
#1— Photo ID with signature
            Dept. of Motor Vehicle ID (NYS or other)– license or non-license ID
            OR
            Another photo ID – such as Military ID, Passport, Sheriff's ID, or                           Department of Social Services card (new ones have photos)
AND
#2— Address verification
            A current utility bill in your name with address
            A check or deposit slip
            A current bill from a credit card company
            Paperwork from social services or Department of Labor,
            or another official agency

            As a last resort, we can fill out a postcard and we will mail it.
            When you receive it, bring it in as proof.
            A PO Box address only will not be acceptable. We need a street address.

Patrons will be asked to fill out an application with their name, address, home and business phone numbers, date of birth, and provide valid driver’s license.

Teens and Children’s applications must be signed by the applicant and a parent/guardian who possesses a valid library card.
